Contract Agreement

This Agreement Agreement is between Sandra Akins, Certified Nutrition Coach, and her Client

Services Provided:

1. Sandra Akins agrees to provide nutrition coaching services to her Client.
2. The nutrition coaching services may include but are not limited to meal planning advice, nutritional guidance, and ongoing support.

Payment:

1. The Client agrees to pay Sandra Akins the agreed-upon fee for the Nutrition Coaching services.
2. The payment shall be made in full before the commencement of the coaching services.
3. All payments are non-refundable. But are transferable.

Client Responsibilities:

1. The Client agrees to provide accurate and truthful information regarding their health history, dietary habits, and medical conditions.
2. The Client acknowledges that the nutrition coaching services provided by Sandra Akins are not a substitute for medical advice or treatment.
3. The Client is solely responsible for their compliance with the nutrition plan and making any necessary adjustments to their dietary habits based on their specific needs.

Confidentiality:

1. Sandra Akins agrees to keep all Client information strictly confidential.
2. The Client understands that Sandra Akins may collect personal information to provide the nutrition coaching services and agrees to collect and store such information.

Termination:

1. Either party may terminate this Agreement at any time with written notice.
2. In the event of termination, the Client will not be entitled to a refund of any payments made.

Limitation of Liability:

1. Sandra Akins shall not be held liable for any damages or injuries arising from the Client's use of the nutrition coaching services or implementation of the nutrition plan.
2. The Client acknowledges that the results achieved from the nutrition coaching services may vary and depend on factors beyond the control of Sandra Akins.

Entire Agreement: This Agreement constitutes the entire understanding between Sandra Akins and her Client and supersedes any prior agreements or understandings, whether written or oral.

By signing below, both parties acknowledge that they have read, understood, and agree to the terms and conditions of this Agreement.
